A family practice, in every sense.
NGC Integrated Healthcare was founded by Dr. Chris Azode and Ngozi Azode — a husband-and-wife team of nurse practitioners who saw a gap in how care was delivered. Medical clinics treated the body. Mental health clinics treated the mind. Few treated the whole person. NGC was built to close that gap.

Dr. Chris Azode
DNP, MSN-Ed, MBA, PMHNP-BCDr. Chris Azode is a board-certified Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC), licensed in Arizona and nationally certified through the American Nurses Credentialing Center (ANCC).
Dr. Chris brings extensive experience across the full spectrum of psychiatric care. He has practiced in inpatient intensive psychiatric units, crisis centers, substance use treatment programs, and child and adolescent settings. That breadth means he has seen — and treated — nearly every kind of mental health challenge, and he meets each patient with the same compassion and careful listening.
In addition to psychiatric care, Dr. Chris also treats acute and chronic medical concerns. He participates in Medicare and Medicaid (AHCCCS) and is authorized to prescribe Medicare Part D medications. NPI: 1306492046.
“Quality care begins here.”
Asked what he hopes to accomplish for his patients, his answer is the same phrase that became NGC’s motto.

Ngozi Azode
MSN, FNP-C & PMHNP-BCNgozi Azode is a board-certified Family Nurse Practitioner through the American Association of Nurse Practitioners (AANP) and a board-certified Psychiatric-Mental Health Nurse Practitioner, with more than 20 years of experience in healthcare and over five years in advanced practice. She earned her MSN from Walden University in 2014.
Before becoming a nurse practitioner, Ngozi spent a decade on the pulmonary medical-surgical floor at Banner University Medical Center — demanding, high-acuity work that shaped her clinical judgment and her bedside manner. As a nurse practitioner, she has cared for children and adults, treating everything from everyday illness to complex chronic conditions.
Her dual certification in family medicine and psychiatric-mental health makes her NGC’s integrated model in a single provider. Ngozi practices holistic, patient-centered medicine. She believes patients should be true partners in decisions about their own health, and she takes the time to listen before she treats. That philosophy — care for the body, mind, and spirit — is what led her to co-found NGC Integrated Healthcare with her husband.
Outside the clinic, Ngozi treasures time with her family — movie nights, reading, and cooking for the people she loves.
